We identify information that may require clarification or correction. The organization determines whether a correction is required, who completes it, and how it is approved. Any authorized correction workflow must be agreed upon in writing.

Reviews use agreed criteria and the organization’s documentation expectations. The scope may include clarity, objectivity, completeness, internal consistency, and recurring documentation patterns.

Feedback may be routed through the organization’s preferred process. The organization decides who receives feedback and remains responsible for employee direction and supervision.

Notereflex provides documentation-focused guidance and may support educational refreshers based on recurring trends. This does not replace required clinical supervision or training.

Appropriate agreements and access controls are established before identifiable records are accessed. Notereflex reviews identifiable notes only through organization-authorized EHR access or another secure platform approved by the organization. Protected information must not be submitted through public website forms, ordinary email, or text messages.

Yes. During a discovery call, Notereflex demonstrates the process with a prepared fictional sample note. No real client records or protected health information are used, and the demonstration is not a pilot.
